The MISD ISD athletic department would like to thank you for taking part in our new process of accepting the required athletic participation paperwork online/electronically. In addition to saving valuable resources, this new automated process will allow us to be more efficient in handling these important documents while providing instant verification that the athletic department at your child’s school has received the necessary paperwork for athletic participation.
IMPORTANT! PLEASE NOTE that all 7th-12th graders have to fill out the Medical History form. You cannot submit the MEDICAL HISTORTY AND/OR PHYSICAL EXAMINATION FORM online. They must be filled out and returned to the campus athletic trainers OR the coach at your child’s campus. Physical forms are available online or on the MISD Athletic website.
PLEASE FOLLOW THE STEP-BY-STEP INSTRUCTIONS FOR SUBMITTING REQUIRED PARTICIPATION DOCUMENTS ONLINE BELOW:
STEP 1. You will need to have the following information available to submit online appropriately:
* Your Child’s LEGAL NAME (as used during registration- must include hyphens if applicable)
* Your child’s ID number (NO LETTERS OR SYMBOLS –EXAMPLE: 12345)
* Emergency contact phone numbers
* Name and contact of primary physician
* Insurance policy information (if possible)
STEP 2. To access the required athletic participation forms for MISD ISD, hold your cursor over the “Electronic Participation Forms” tab. You will get a drop down list of all required forms.
STEP 3. To begin, click on the form name and fill out the information requested (you must have your students ID number available as it is required on each form).
* DO NOT LEAVE BLANK SPACES
* Write N/A in any space that the information is unknown
STEP 4. SIGN THE DOCUMENT(S). To sign each document:
* Click inside the signature box and hold your mouse down,
* While holding down the mouse “sign “your name in the signature box. If you make a mistake and need to start over click on the refresh icon next to the signature box.
If you are using an IPAD, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus.
REPEAT STEPS 1 – 4 FOR ALL REQUIRED DOCUMENTS SHOWN. ONCE YOU COMPLETE A DOCUMENT IT WILL DISAPEAR FROM THE LIST.
IMPORTANT! To submit online appropriately, you AND your child MUST fill out and sign in ALL REQUIRED SPACES. Forms will not be accepted electronically without everything filled out by the student athlete and you the parent.
Once you have filled out all of the information on each page you will have the opportunity to print the document.
You will receive a confirmation email when each form is submitted. If it doesn’t show up in your in inbox please check your junk mail folder.
